The Elder Care Continuum

Life care planning is a holistic approach to the practice of elder law that helps families address the challenges associated with the disability and/or chronic illness of a loved one. Our goal with respect to life care planning is to foster and maintain the health, safety, quality of life, and overall well-being of our elderly clients.

We take a holistic, team approach the practice of life care planning. Our Certified Elder Law Attorney, Care Advocate, Benefits Specialist, Estates Paralegal, and Client Services Coordinator work together to help families make informed choices about every aspect of their loved one’s physical and emotional well-being. This involves identifying current and future care needs, locating the appropriate types of care, and ensuring that care is of the highest quality.

We forge strong, long-lasting relationships with our clients and their families. In fact, many clients come to view us as part of their family. Similarly, we consider our clients to be members of our family.

We use what is called the Elder Care Continuum to help our clients and their families understand the natural progression of aging and how it impacts an elder’s health, housing, mobility, and finances. During your initial meeting with us, we’ll discuss your loved one’s place on the Elder Care Continuum. Gaps can then be identified and solutions proposed for closing those gaps.

The Life Care Plan

Our team works closely with clients to create customized, holistic life care plans. These plans define, prioritize, organize, and mobilize each and every aspect of a client’s care. Our plans incorporate all of the traditional elements of elder law, including estate planning, asset protection, and the procurement of Medicaid and other benefits to pay for care. In addition, our plans go well beyond the traditional approaches and include provisions for care coordination, education about health care and financial matters, care advocacy, crisis management, and other valuable services.

While every one of our life care plans is unique, in general they are designed to accomplish the following objectives:

  • Ensure our elderly clients receive the care they need to maintain the quality of life they desire
  • Find public and private sources to help pay for the level of care required
  • Provide the peace of mind that comes from making the right choices to ensure the best possible care is received while protecting the family’s financial resources
